Introduction

The Home and Community Care (HACC) Program provides access to a Dementia Link worker from Alzheimer's Australia for drop-in consultations. These consultations provide:

Information to connect you with community support and networks
In home services and medical services
Advocacy with a GP, family and or services
Information regarding Residential Care services
Information sessions for understanding the diagnosis
